Chilmark Board of Health Minutes
June 7,  2006

Attendees: Mike Renahan, Katie Carroll, Jan Buhrman, Sylvia Yeomans, Reid Silva, Chris Alley, George Sourati, Warren Doty, Lenny Jason Jr., Allen Healy, Kent Healy, Peter Wells, Jay Walsh

The minutes of May 3, 2006 and May 17, 2006 were approved as corrected.

Middle Line Road Project B Two members of the sub-committee were present to discuss the Middle Line Road Project, Warren Doty and Lenny Jason Jr.  Warren and Lenny discussed the proposed layout and if each building required a separate septic system.  The original plan proposed one well and one septic system for each cluster of buildings.  They asked if one well could service all buildings and if it was considered a public water supply.  Prior to speaking to the Board of Health, they spoke to the engineer, Kent Healy for advice regarding septic system locations.  Kent informed them that if they had one septic system per cluster, they would still require a separate septic tank for each building. Mike is concerned about the abundance of clay in the area.  He thinks more test holes may be required.  Mike also expressed his belief that if you own a house and sell a house you should also own and sell the septic system.  Mike asked who pays for maintenance and repair of a failed system if a cluster of buildings are sharing it. Mike also pointed out that the area is known to have problems drilling wells. Both Warren and Mike would prefer separate septic systems for each building.  All single residences should have their own septic system.  More perc tests can be done if necessary even if they are out of season.  

Allen Healy B The Board agreed to inspect the freezer Mr. Healy stores his meat in, located in his barn.  It is the understanding of the Board, that Mr. Healy will be selling inspected, frozen meat to the public, from his farm and at the farmers market in West Tisbury. A letter to confirm inspections of the freezer will be sent to John Powers at the West Tisbury Board of Health.

Roseline Glazer (21-78) B The owner has proposed making the existing guest house an office and installing appliances in the existing office to change it to a guest house. Mike requested a site visit prior to making a determination on the disposal works application.  He will accompany Sylvia while doing a bedroom count at the site, and Sylvia will research the minutes to determine why the four bedroom disposal works application #03-31 was denied on September 17, 2003.

Forbes Smiley (20-41) B The Board reviewed a letter from Kent Healy regarding the status of this septic system construction.  Sylvia will send a letter to Mr. Smiley instructing him to make the necessary repairs as follows:
1)  Expose the leaching bed and inspect it to determine work needed to repair it.  2) Locate and test any 2" PVC piping remaining between the pump tank and the D-box.  3) Hire a qualified contractor to repair the piping from the pump tank to the D-box, install an effluent pump and alarm system in the pump tank, reseal all risers on the tanks.  
4)   Submit an as-repaired plan and inspection certification to the Chilmark Board of Health.  All work must be completed within 60 days of receipt of the letter.  A second letter will be sent to Mr. Smiley informing him of the low interest loan program available to qualified Chilmark town residents.  

Sheriff=s Meadow Foundation (22-5.3) B The Board approved a revision to disposal works permit #5-28.  The leaching area was relocated due to a change in the location of the house.

Honker & Sons LLC / Bill Murray (35-1.2) B This disposal works design is in the coastal district and requires two (2) local variances as follows; 1) A reduction in separation between leaching area and wetlands from 200 feet to 83 feet.   2) A reduction in separation between leaching area and Squibnocket pond from 500 feet to 177 feet.  The Board has instructed Sylvia to research the missing inspection report.  If renovations and remodeling are done to this building or out-buildings, the Board wants advanced treatment used.  This will be continued at the June 21, 2006 meeting.

Pacer Nominee Trust / Barbara Foster (35-1.15) B This is a shared septic design, main house and guest house.  The engineer must submit a site drawing showing that the property will support two Title 5 compliant systems.  If this is satisfied, a shared system can be designed.  This will be continued at the June 21, 2006 meeting.

Barbara Elias (21-25) B This parcel failed a septic inspection on March 16, 2006.  The Board approved a repair permit to replace the septic tank.  The deadline for repairs has been extended from June 12, 2006 to June 30, 2006.


Board of Health Minutes
June 7, 2006

Jeff Bezanson (27.1-160 & 161) B The Board approved a well permit application for this parcel.  The owner will be requested to send a letter acknowledging he is aware that the separation between well and leaching area is less than the 150 feet required by Chilmark regulations.

Jared Stammell (23-7) B The Board will approve this disposal works application if one of the following options is satisfied; 1) The cesspool serving the main dwelling is inspected and upgraded, or 2) A new septic design is submitted for the existing main dwelling as well as the guest house.

Raphael Teller (24-17) B  The Board approved this disposal works application for a four (4) bedroom house.  This repair will replace a 750 gallon septic tank with a 1,500 gallon tank, and replace a failed leaching pit with a new
Title 5 infiltration leaching field.

Aileen & David McGlade (30-55) B The Board approved this disposal works revision #5-13B to include attaching the garage with a detached bedroom to the existing design.  This is a total of 6 bedrooms.
George Rivera (17-21.1) B The Board approved this disposal works application for a proposed five (5) bedroom house. .

Windfall, LLC (25-138) B The Board approved this disposal works application for a three (3) bedroom single family dwelling.

Jody Newman & Martin Rosenthal (8-19) B This house must be inspected next week for repairs imposed with the  human habitation inspection orders.

MV Hill (33-115) B The Board observed an outside shower was installed at this parcel.  Get the status of the building project from the Building Inspector.

Ralph Seferian (20-20) B Research status of well installation and any response to the letter sent March 24, 2006.

The Board wants to see copies of any water sample reports sent to DEP by Menemsha Water Company.  Call Lois Norton regarding water sample reports for Menemsha Water.

Joyanne Flanders (19-9) B The Board approved this tent permit.

The following sewage disposal system inspection report was received:
John & Caroline Moore (25-27) B passes

The following Aas-built@ letter was received:    
Mark London (20-55) B John Keene Excavation installed this upgrade to replace a 750 gallon septic tank with a new 1,500 gallon septic tank and tie it into the barn addition.
